Absence Policy

Purpose:  Medical documentation from your physician is required for an absence of three or more days.  The documentation may be faxed to 630-829-6035.  After receipt of the physician's note, the staff will inform the Vice-President of Student Life of the extended absence.

The office of Student Health Services offers a nursing evaluation of sick students and can provide a student with a "Notification of Illness" after evaluation.  A Notification of  Illness is not intended to be an excused absence, only a verification that the student was seen in Student Health Services.  Instructors have individual attendance policies; the student is encouraged to communicate with their instructors regarding their absences.

Required Medical Documentation for Extended Absences of 3 or more days 
Student must provide medical documentation and release from their physician before returning to school and resuming activities. The medical documentation must include:
1.  Dates of absence
2.  Date released to return to school, activities and residential life, if applicable
3.  The name of the physician, contact information, and must be signed by the physician

Additional Note
It is not the staff's responsibility to make arrangements for missed classroom assignments, materials, etc. Students are encouraged to contact instructors regarding work covered in their absence.

Health Tip
Hand washing is your best defense against colds, flu, and other viruses.
Remember to wash hands after using keyboards and the mouse, and especially before eating meals.
